The grammar patterns たら最後 and たが最後 both convey that once a certain action (A) is taken, a negative outcome (B) will certainly happen. The result (B) is always an undesirable situation that is seen as an unavoidable consequence of (A). Common English equivalents include 'once (A), then (B)', 'as soon as (A) happens, (B)', or 'if (A) ever happens, then (B)'.
The particles たら and が in these structures function as connectors. たら keeps its familiar conditional meaning of 'if' or 'when', and が introduces a contrast, much like 'but' or 'however'. This creates a subtle difference: たが最後 implies '(A happens), but the end result is (B)', whereas たら最後 suggests 'if/when (A happens), the end result is (B)'. Despite this minor distinction in nuance, the two forms are considered completely interchangeable in practice.
- この秘密を話したが最後、もう元には戻れない。Once you tell this secret, there's no going back to the way things were.
- このゲームを始めたが最後、お金がどんどんなくなるよ。As soon as you start this game, your money will just keep disappearing.

Due to its placement before the comma, it might seem like 最後 is grammatically linked to が or たら. In reality, it logically relates more to the second part of the sentence (B), emphasizing that this will be the 'final result' or 'ultimate outcome' of the action.
Caution
In some cases, the second clause (B) may be left out entirely if the negative consequence is clearly understood from the context.
- 彼にマイクを渡したら最後だ。It's all over once you hand him the microphone. (He'll talk/sing forever)
- 親が子供を叱らなくなったら最後だ。It's the end of the line once a parent stops scolding their child. (It means they've lost hope)
